Employee and Contractor Health Attestations

Employees and contractors are required to attest to their health status by completing a brief online questionnaire before entering our buildings or in the field. The attestation is required each day prior to reporting to our facilities or traveling to business client locations.

This user-friendly tool is available from a corporate or personal device such as a laptop, tablet, or smart phone. Mobile kiosks will also be available as in company lobbies or at suite entrances in shared locations. It will only take a couple of minutes to complete before coming to work or accessing an office.

You will be asked on-site if you have completed the attestation. Those who have not completed the questionnaire will be asked to do so before going through the temperature check station via lobby kiosk. At facilities with self-check temperature screening, leaders should ask their employees each morning if they completed the attestation before coming to work.

The new process grants access to our facilities after you submit the daily attestation with responses that confirm:

• You have not been diagnosed with COVID-19.
• You are not experiencing symptoms of COVID-19.
• You have not had exposure to someone who has been diagnosed with COVID-19.

Temperature checking will continue as employees and contractors enter buildings.

You’ll then have facility access for the entire day and you will not need to complete the attestation again until the next day you will be reporting to a facility.

The new process does not include visitors – who will continue to check in at the facility’s security desk and complete an attestation form at that time.

Accessing the web link tool using corporate or personal device
The steps to complete the attestation are the same for every device:

Step One
Enter the web address https://bcbsm.appiancloud.com and it will take you to the login screen. 

Log into the tool using your computer login email address and password.

Step Two
Select the facility you will be reporting to.

Step Three
Complete your profile.

Step Four
Answer the attestation health questions and hit “Submit.” Steps two, three, and four will only need to be completed once unless your profile or responses to the health questions change in the future.

Step Five
If your answers to the attestation health questions have not changed from the previous day, you will only need to hit “Submit.”
